Kevin Helms, MD
Dr. Helms joined our team in 2016 as Medical Director. He received his medical degree from Indiana University School of Medicine in 1997. His residency for internal medicine was completed in 2000 at Methodist Hospital. He was Program Director from 2004 to 2007 at Methodist Hospital. Dr. Helms served as the medical director for the IU North Transitional Care Program, American Senior Communities Allisonville Meadows, The Stratford at the Village of West Clay, Kindred Bridgewater and Kindred Eagle Creek. Dr. Helms is currently a Hospitalist with the Ortho Indy Hospital. Dr. Helms is a member of the American Medical
Association, Society of Hospital Medicine, American
Medical Directors Association and the Indiana State
Medical Society. Dr. Helms is licensed to practice medicine in the state of Indiana.
Olivia Cronin, MD
Dr. Cronin joined the Grace At Home team in 2014. She was appointed MEDICAL DIRECTOR effective July 1, 2016. Dr. Cronin graduated with honors from Loyola University in Chicago with her Doctor of Medicine Degree. Her residency was in Emergency Medicine at Indiana University in Indianapolis. She brings great clinical skills and leadership to the Grace At Home team. Dr. Cronin is licensed to practice medicine in the State of Indiana.
Lynn Collins, FNP-C
Lynn Collins has been a Family Nurse Practitioner since 2016, with a focus on geriatrics.
She received her Bachelors of Science in Nursing from Bellin College in Green Bay, WI, and her Masters of Science in Nursing - FNP from the University of South Alabama in Mobile. She is board certified by the American Academy of Nurse Practitioners.
Lynn is married and has four daughters. When she’s not working, she enjoys coaching youth sports, traveling, and hanging out with friends and family.
Katlyn Gahagan, PA-C
Physician Assistant Katlyn Gahagan began practicing in 2022. Her areas of clinical interest include primary care and getting to know patients and their families. She says, “I specifically enjoy working with the geriatric population as I value the wisdom and experiences they share.” Katlyn is board-certified by the National Commission on Certification of Physician Assistants. She is also certified in Basic Life Support, Advanced Cardiovascular Life Support and Medication-assisted treatment.
As an Indy native, Katlyn is excited to serve patients in the Indianapolis area. She says, “I strive to treat my patients in a way I would treat my own family members.”
Katlyn received her Bachelor of Science in Exercise Science at the University of Evansville in Evansville, IN, and her Master’s in Physician Assistant Science through the University of Evansville Physician Assistant Program. She is a member of the Indiana Academy of Physician Assistants.
In her spare time, she likes to read, play some volleyball, and spend time at the lake with family.
Nikki O'Rourke, NP
Nikki O'Rourke has been a board-certified family nurse practitioner since 2009. Her years of experience are in the fields of Internal Medicine and Cardiology in the adult and geriatric populations. She has been with Grace at Home since 2015. She received her Master's of Science in Nursing from The University of Indianapolis with continued Post Master Studies at Northern Kentucky University. Nikki is a highly qualified and compassionate Nurse Practitioner who brings a wealth of knowledge to the team. Her vision is to provide grace to her patients and their families through standards of care, medicine, education and compassion. She strives to meet all expectations and goals set forth. Nikki is licensed to practice medicine in the state of Indiana.
Maggie received a Masters in Physician Assistant studies from Butler University. She is certified in Family Medicine and has over six years’ experience in acute and emergency medicine as well. She has an energetic spirit that she brings to delivering compassionate healthcare. You will find her style approachable and empathetic as she provides Grace At Home patients’ with exceptional healthcare. She services patients in the Noblesville, Sheridan & Westfield area as well as a group of patients in 46268. Maggie is licensed to practice medicine in the state of Indiana.
Margaret (Maggie) Corbett, PA-C
Beth Ann Taylor, NP
Beth Ann is a board certified nurse practitioner with over 30 years of experience, specializing as a Geriatric Nurse Practitioner. She has an easy going personality and is a great resource for her patients. Her years of experience are in the fields of house calls, primary care and urgent care as well as an educator. She has experience treating complex patients with extensive neurological disorders, chronic pain, psychological problems and palliative care. Beth Ann’s territory is Bartholomew, Tipton and Howard County. Beth is licensed to practice medicine in the state of Indiana.
Zachary (Zach) Eppert, NP
Zach comes to us with over 10 years experience as a Clinician. His commitment to quality and continuity of care has been a huge asset in his nursing career. Zach's background consists of working in several areas of medicine including Emergency, Oncology, Med/Surg, Hospice/Palliative and Pain Management. He has held several leadership roles such as Clinical Manager, Care Plan Developer and was responsible for the Education and Training of his clinical team. His hospital based experience, especially with the VA and Community North, make him a very valuable asset to the Grace At Home Team. Zach served as a Corporal/Team Leader in the United States Marine Corps prior to his healthcare experience. Zach’s territory will be focused on the East side of Indianapolis including Greenfield and New Palestine. Zach is licensed to practice medicine in the state of Indiana.
Joel Chodakowski, NP
Nurse Practitioner Joel Chodakowski began practicing in 2022 with a focus on family medicine. He is board-certified by the American Association of Nurse Practitioners.
Joel received his undergraduate degree at Marian University in Indianapolis, IN, and a Master of Science in Nursing at Frontier Nursing University in Hyden, KY.
On a personal note, Joel enjoys woodworking, camping and spending time with his kids.
Molly Malina, NP
Nurse Practitioner Molly Malina’s focus is on family medicine, with clinical interest in primary care, the geriatric population and emergency medicine. She is board-certified by the AANP, of which she is a member; she’s also a member of the Coalition of Advanced Practice Registered Nurses of Indiana.
Molly received her Bachelor of Science in Nursing at Indiana Wesleyan University in Marion, IN, and she also graduated from Wesleyan as a Family Nurse Practitioner.
On a personal note, Molly enjoys traveling, spending time with family, being outdoors, staying active and reading.
Donna Blake, NP
Donna Blake is a Nurse Practitioner with a focus on Family Medicine and special interest in adult care. She has been practicing since 2013.
Donna has a Bachelor of Science in nursing from Indiana University–Purdue University Indianapolis (IUPUI), where she also earned a master’s degree. She is board-certified as a Family Nurse Practitioner by the American Association of Nurse Practitioners (AANP) and is a member of the Coalition of Advanced Practice Registered Nurses of Indiana (CAPNI).
Away from her job, Donna loves animals, being outdoors, the beach and the sun. She plants a garden each year and tries to eat a healthy and balanced diet, though she loves baking and chocolate! She has two adult children, a daughter and a son.
Natasha Theiring, PA-C
Natasha Theiring, PA-C is a member of our Village at Home care team and began practice in 2016. She holds keen interests in Family Medicine and Endocrinology.
Natasha’s educational journey began at Butler University in Indianapolis, IN, where she obtained a bachelor’s degree in Health Sciences. There she continued her training completing a master’s degree in Physician Assistant studies. Natasha is certified by the National Commission on Certification of Physician Assistants.
Outside of work, Natasha enjoys exercise and playing pickle ball. Additionally, she likes to travel and cook. At home, she spends time with her family including a black goldendoodle named Rollins, who loves to snuggle and meet new friends, both human and furry.
